Research suggests that more than half of people who’ve had a concussion experience some kind of visual or eye-movement problem, even when a standard eye exam says their vision is a perfect 20/20. A “SIMPLE” TASK IS NEVER REALLY SIMPLE
Here’s the bigger picture. Almost nothing your brain does happens in isolation — and vision is a perfect example.
Take something as ordinary as reading the sentence you’re reading right now. To make it happen, your two eyes have to aim at the same word together, focus at exactly the right distance, and make a series of quick, precise jumps along the line. Your brain filters out everything in your periphery it doesn’t need, while your balance system quietly keeps your head steady so the words don’t bounce. Then your brain takes all of that raw input and turns it into meaning. Dozens of regions, firing together, in a fraction of a second — for a task you’d call “simple.”
You never notice any of this when it’s working, because it’s automatic. After a concussion, any link in that chain can falter. And because the skill itself was invisible to you, you don’t experience it as “my eye teaming is off” — you just experience reading as suddenly, bafflingly exhausting.
Here’s part of what’s going on under the hood. A concussion can disrupt the brain’s usual, well-worn pathways for these tasks. The brain is remarkably adaptable, so it reroutes — but those alternative pathways aren’t as fast or efficient as the originals. The job still gets done, just with more effort and a slight lag. Eye teaming is your two eyes’ ability to work together as a coordinated team — converging (turning inward) to focus on something close, and diverging (turning outward) to focus on something far away — so that what you see stays single, clear, and comfortable. After a concussion, that coordination can slip, which leads to blurry or double or overlapping vision, eye strain, and that “I have to reread every sentence” feeling.
Focusing:
Your eyes constantly shift focus as you look from your laptop or notebook down close, back up to a whiteboard across the room, then back down again — automatically, all day long. When that focusing system is disrupted, those shifts go blurry for a beat and your eyes fatigue quickly. People often describe it this way: “things are blurry at first, but I can eventually make them clear if I really work at it.” So you end up living in a world of intermittent blur — vision that drifts in and out of focus, clear only as long as you keep forcing it.
Tracking and scanning(saccadic eye movement):
Reading a line of text, following a moving ball, scanning a room — these all rely on precise eye movements. After a concussion they can become jumpy or effortful, so you lose your place, skip words, or feel worn out after just a few minutes of reading.
Peripheral awareness and balance:
Vision isn’t just what’s straight ahead — it’s also taking in the edges: the car pulling up beside you, the doorway, the child darting into view. After a concussion, the brain can struggle to process all of that peripheral input. Part of what’s happening is that the brain has trouble sorting what’s an actual threat from what’s harmless background, so every bit of motion at the edges — a passing shopper, a flickering sign, movement in the next aisle — can register as something to react to. That’s exhausting, and it’s part of why busy, cluttered environments suddenly feel so overwhelming.
It also doesn’t stop at vision. Your eyes and your vestibular (inner-ear balance) system are close partners — together they tell your brain where you are in space and keep the world feeling steady as you move. When peripheral processing is overloaded, that partnership gets thrown off too, which is why busy or fast-moving situations can leave you dizzy, unsteady, or queasy, and throw off your sense of space — even when nothing’s physically wrong with your ears.

Here’s why this gets missed so often: the symptoms rarely announce themselves as vision issues. Instead, they show up as:
- Headaches, especially with reading or screen time
- Trouble concentrating, or “brain fog” that gets worse as the day goes on
- Feeling drained or foggy after using a computer or phone
- Losing your place when reading, or rereading the same line
- Dizziness or queasiness in busy, visually cluttered places
- Motion sensitivity — riding in a car, scrolling, fast-moving video
- Sensitivity to bright light, glare, or flickering screens
- Feeling anxious or overwhelmed in environments that never used to bother you
